Sounds like, for what ever reason, the bearing was not rotating. I would review your installation procedure(s)to determine if proper installation of the sleeve, bearing is the root cause. Additionally, the pump drive could be misaligned relative to the shaft by design or "other" and cause binding of the sleeve and bearing.
