Check out Lowell W. Foster, considered to be "the father of GD&T. GD&T was originated for use in the manufacture of military hardware back in the 60's.

In the past I have had success using star washers when fastening to aluminum, as the material is soft enough to allow the star washer to "dig in". Personally I wouldn't recommend using it on anything larger than a 1/4" fastener, and I probably wouldn't recommend using it at all on steel or stainless steel. As for the flat washers, if the bolt is a tight fit in the hole, you shouldn't need to use a flat washer under a split lock washer.
